A Community Care Navigator from Hilton Head Regional Healthcare comprised of Coastal Carolina and Hilton Head Hospitals will be present in the Live your Life Well Center at the fitness center during designated hours.  The Community Care Navigator may provide the following services to residents, free of charge:

• Obtaining blood pressures

• Recording blood pressures for Resident to submit to care provider

• Obtaining height, weight, and BMI (Body Mass Index)

• Identifying resources for nutrition education

• Providing guidance in identifying medical resources in the area

• Providing assistance in scheduling appointments with physicians and healthcare facilities.

The Community Care Navigator cannot provide diagnosis or treatment; but can assist in identifying physicians, healthcare facilities and other resources in the community for residents seeking diagnosis or treatment.
