A Community Care Nurse Navigator from Novant Health, comprised of Coastal Carolina and Hilton Head Medical Centers, will be present in the Live your Life Well Center at the fitness center during designated hours.  The Community Care Nurse Navigator may provide the following services to residents, free of charge:

• Vital Signs check

  o     Blood Pressure

  o     Heart Rate

  o     Oxygen Saturation

  o     Body Mass Index

• Obtaining blood pressures

• Recording blood pressures for Resident to submit to care provider

• Obtaining height, weight and BMI (Body Mass Index)

• Identifying resources for nutrition education

• Guidance on finding local healthcare providers

  o     Assistance in locating doctors across various specialties

The Community Care Nurse Navigator cannot provide diagnosis or treatment; but can assist in identifying physicians, healthcare facilities and other resources in the community for residents seeking diagnosis or treatment.
